Initial Consultation:

We want to answer as many of your questions that we can.   Please prepare for this by bringing the following:

1. Health Insurance Card(s)/Information
2. Workers Compensation paperwork
3. A list of all medications you are currently taking
4. A copy of X-rays, CT, MRI films, along with the written reports
5. A list of your questions/concerns.
